Breen’s cognitive difficulties, in whatever form they take, are just as likely to be due to his extreme prematurity, and all that he has endured, as they are to be due to this specific PVL patch. Meaning, a kid who has been intubated for a long time, got NEC, has a short gut, has been frequently sedated and medicated, living in a box with limited interaction and stimulation, living in a noisy and disruptive environment, went on too many helicopter rides, multiple surgeries, constantly being poked and prodded, etc etc… We’re talking about near constant trauma and overstimulation, it’s difficult to be able to predict how much this will impact the rest of his life. The PVL isn’t great news, but Dr. O explained that it’s simply one part of a myriad of difficulties that Breen has experienced that will dramatically shape the quality of his life.
  2. At this point, his being intubated is potentially more damaging to his future cognitive abilities than the PVL. She explained in the first year of a baby’s life, their physical movements and developmental abilities are directly correlated to them building strong neuro-pathways which will serve as the foundational infrastructure on which all of his future cognitive development is built. The more he moves now, the better he’ll think later. That tube significantly hinders his movements, something he’s been struggling with recently, and getting the tube out will free up his movement, which will allow his brain to grow.
